SUBSTANCE ABUSE EVALUATION (CLINICAL)

Court-ordered substance abuse evaluations conducted live via Zoom by appointment only. 

This is a clinical assessment (not counseling or treatment) used to determine substance use concerns and court-appropriate recommendations.


SCHEDULE EVALUATION

EVALUATION OVERVIEW

The Substance Abuse Evaluation is a non-treatment clinical assessment conducted by a qualified, licensed professional. 

The evaluation includes review of required screening tools, a clinical interview, and written recommendations when required by the referral source.

This evaluation is not counseling and does not include ongoing treatment services.


WHO THIS EVALUATION IS FOR

This evaluation is for individuals who have been ordered by a court, probation department, or referral authority to complete a substance abuse evaluation or clinical assessment.



  • Court-ordered substance abuse evaluations  
  • Probation or parole requirements  
  • Individuals required to obtain an evaluation following a positive UA  
  • Individuals required to determine appropriate treatment level  
  • Adults able to attend a live Zoom appointment  



REQUIRED ASSESSMENTS (MANDATORY)


All required screening assessments must be completed prior to your scheduled evaluation appointment. 

These assessments are included in the evaluation fee at no additional cost.

Failure to complete all required assessments may delay or cancel your evaluation.


THE PROCESS

  • Step 1: Complete all required assessments (MAST, DAST, and SALCE).  
  • Step 2: Schedule your evaluation appointment.  
  • Step 3: Submit payment to confirm your appointment.  
  • Step 4: Receive booking confirmation and payment receipt by email.  
  • Step 5: Receive portal access instructions within 24 hours.  
  • Step 6: Complete required intake forms and upload a valid photo ID.  
  • Step 7: Attend live Zoom evaluation (approximately 30–45 minutes).  
  • Step 8: Evaluation recommendations are completed and submitted as required within 48 hours.

MAST (Alcohol Screening)

The Michigan Alcohol Screening Test (MAST) is used to evaluate alcohol-related behaviors and risk patterns.

The SALCE assessment helps determine substance use patterns and potential treatment needs.


Instructions:

  1.  Click the link below 
  2. Select “SALCE Interactive Survey (English)” 
  3. When prompted for an ID, enter the last 4 digits of your Social Security Number 
  4. Complete all questions accurately 
  5. Click Submit when finished

REPORTING & IMPORTANT NOTES

  • Evaluations are conducted by appointment only.  
  • Required assessments must be completed prior to the evaluation appointment.  
  • Reports are prepared in accordance with court or probation requirements.  
  • Results do not guarantee placement into or exemption from any treatment program.  
  • Final decisions regarding education or treatment requirements are determined by the court or supervising authority.
