RELAPSE PREVENTION PROGRAM (AFTERCARE)

Court-ordered relapse prevention and aftercare program delivered live via Zoom (Virtual only – no in-person sessions available)

Who This Program Is For

 This program is for individuals who have been ordered by a court, probation department, or referral authority to complete a Relapse Prevention or Aftercare program.


  • Court-ordered relapse prevention participants 
  • Individuals required to complete ongoing recovery or aftercare services 
  • Participants transitioning from treatment or ordered directly into relapse prevention 
  • Adults able to attend live Zoom sessions from home

PROGRAM OVERVIEW

 The Relapse Prevention (Aftercare) Program is a structured outpatient support program conducted entirely online through live Zoom group sessions. The program focuses on identifying triggers, strengthening coping strategies, accountability, and long-term recovery planning while allowing flexibility in session attendance. 

PROGRAM STRUCTURE

 

  • Total sessions required: 12 group sessions
     
  • Session length: 2 hours per session
     
  • Attendance limit: 1 session per week
     
  • Typical completion timeframe: 12 weeks
     
  • Maximum completion window: 15 weeks from booking date
     
  • Open enrollment: No fixed start date
     
  • Program language: English only
     
  • Certificate of completion issued upon successful completion

SESSION SCHEDULE & FLEXIBILITY

Participants are provided access to multiple weekly session times and may select sessions based on availability, as long as attendance limits are followed.

  • Weekly Sessions Available:


  • Monday – 7:00 AM
     
  • Monday – 7:00 PM
     
  • Tuesday – 7:00 AM
     
  • Tuesday – 7:00 PM
     
  • Thursday – 7:00 AM
     
  • Thursday – 7:00 PM
     
  • Saturday – 7:00 AM
     
  • Sunday – 7:00 PM
     

Participants may attend one session per week only and may choose different days and times each week.


ENROLLMENT & COMPLETION RULES

  •  Relapse Prevention operates as an open-group program
     
  • Participants may begin attending sessions immediately after completing all enrollment requirements
     
  • Participants must attend their first session within seven (7) days of the booking date (initial payment date)
     
  • All 12 required sessions must be completed within 15 weeks from the booking date
     
  • Failure to meet attendance or completion timelines may result in non-completion or termination from the program

REGISTRATION & PAYMENT PLAN

 

The total cost of the Relapse Prevention Program is $240. A flexible payment plan is available to support accessibility and court compliance.

  • Initial payment at enrollment: $80
     
  • Monthly payments: $80 per month for 2 consecutive months
     
  • Total payments: 3 payments of $80
     

Participants must remain current on payments to maintain active enrollment. Missed or delinquent payments may result in suspension or termination from the program.

Attendance & Certificate Policy

 Attendance is mandatory for all scheduled sessions attended. Participants must arrive on time, remain on camera, and actively participate for the full session duration.


Certificates of completion are issued only after:

  • Attendance is verified
     
  • All 12 sessions are completed
     
  • All program and payment requirements are satisfied
     

Monthly progress reports are provided to the participant’s Community Supervision Officer (C.S.O.) or referral authority when required.

What Happens After You Register

 

  • Step 1: Complete registration and initial payment
     
  • Step 2: Receive intake instructions and required forms
     
  • Step 3: Complete Practice Better intake documentation
     
  • Step 4: Receive Zoom access to weekly Relapse Prevention sessions
     
  • Step 5: Begin attending sessions (first session must be attended within 7 days)

 

Praxiax provides administrative and technology support services for independent education and treatment providers. Praxiax does not provide legal, medical, clinical, or therapeutic services.

Independent providers using Praxiax are solely responsible for instruction, licensure, treatment delivery, and regulatory compliance.
